Case Study

65/ F underwent bilateral TKR 9 months back
C/O severe low back pain radiating to both legs with difficulty standing and walking for more than 5 min post surgery.

MRI spine showed mild L5/S1 lumbar canal stenosis.

Did stress X-rays which revealed grade 2 L5/S1 Listhesis , that explained her severe claudication symptoms.

We did L4 to S1 instrumentation and decompression. Symptoms dramatically improved post surgery.

1. Few Pts has both back and knee issues. We need to understand in depth cause of pain before we do any surgical intervention.

2. Mobile Listhesis does cause dynamic canal stenosis with claudication. MRI may not reveal dynamic compression. Stress views are helpful in such cases.
